I don't think anyone knows for sure... I'm not familiar enough with the stress testing required (I worked with smaller banks that weren't subject to that level of testing). The Federal Reserve has ordered a review of how the bank was supervised - like were there warning signs sooner that the bank was in trouble and the regulators missed it.
